Did something happen to CV Mutant while you were working with it? CV Mutant has been thoroughly tested, so it must've been something that's really hard to find. Please think very carefully about what happened and write it all down. Every bit of information helps, even information that might not seem relevant. If you have a song- or patch file that can help to reproduce or clarify the situation, please include that also.
